All fees are due upon session ending.  Check or Cash are the forms of payment accepted.  You may email transfer a money order to dwight@dthompson.org - but you must do this prior to your session beginning.  Please let your therapist know prior to doing this.
CANCELLATION POLICY:
24 Cancellation Policy is in effect, failure to cancel will result in a $90.00 charge to your session in the Kemptville office only.
By submitting this form you are responsible for all fees and payment of sessions due at the end of each delivered therapeutic appointment.  
THIRD PARTY INSURERS & GROUP BENEFITS:
Please ensure you check with your insurer prior to coming to the session for coverage.  Many clients fail to do this, and some benefit providers will only cover services provided by a clinical psychologist or a social worker (MSW) who is supervised by a clinical psychologist.  Please ensure you do this, not following through can limit the potential for your therapy to be useful.

Dwight will send/email you a copy of the psycho-social form used for gathering information to assist you in a timely manner.
